About Northern Case Management

Bridge Case Management

NCM have been established since 1997, founded to support people with acquired neurological damage (i.e. brain & spinal cord injury). In 1997 this level of support was almost unheard of but has since become an integral and incredibly important element of supporting people with acquired neurological damage.

We endeavour to provide a non-discriminatory service to people with acquired neurological damage i.e. brain and spinal cord injury for all ages from children to older people in a domiciliary setting. Our goal is to empower them to develop their full potential and enrich their lifestyle through providing a comprehensive client focused service.

NCM work closely with the client and their family or representative to tailor support packages for clients in their own homes and in the community.

We continue to meet the increasing requirement for case management services and now boast a multi-disciplined team of 8 case managers plus support staff and manage over 200 support workers on behalf of clients.     We are regulated by the Care Quality Commission (CQC) and have been rated ‘Good’.
