Our expanding nationwide team of case managers with diverse backgrounds and expertise ensures we can put forward the right candidate for each client that fits their specific needs.

Our Complex Care team is led by highly experienced registered nurses, who not only provide exceptional clinical leadership, but also have deep, practical insight into the day-to-day realities of case management. Our Occupational Therapy team is led by award-winning professionals and built on experience, clinical excellence, and a deep understanding of case management needs. Every referral is handled with efficiency and care, ensuring that clients receive high-quality, evidence-based interventions that promote recovery, independence, and wellbeing.

Unlike standard OT services, Breakthrough OT is designed to complement and integrate seamlessly with case management and complex care services, ensuring a truly holistic approach. The network focuses on goal-centred interventions, delivering personalised support that adapts to each client’s rehabilitation journey.
