Integrated Case Management has been formed to offer a fresh approach to the delivery of person-centred case management in the south of England putting the needs, wishes and expectations of our clients first.
Our case management service has been developed to provide person-centred support to clients and to achieve the best outcomes by working in a closely integrated manner with all of the people involved, including family, carers and the wider professional team. We set realistic and achievable goals with our clients to ensure that they reach their maximum level of independence.
Our Case Managers
We strive to provide continuity of care and intensive support for clients who are in a complex and often challenging environment. Our consistent and caring support is provided throughout the traumatic journey of recovering from a life-changing injury to improve the client’s standard of living through the design, implementation and monitoring of an individually tailored rehabilitation plan.
Our case managers, who have a minimum of seven years’ post qualification experience, bring their wealth of knowledge, commitment and passion for supporting clients to the rehabilitation journey, enabling our clients to achieve their goals. By setting realistic goals we enable our clients to address the new challenges presented to them and to look forward with a sense of positivity.
The experience and expertise of our case managers enables them to devise and co-ordinate the most appropriate and effective rehabilitation and care solutions, ensuring that all involved with the client work together in an integrated way to achieve the best possible outcomes. Our case managers are highly responsive to clients and instructing parties, as well as readily adaptive to change, providing excellent communication and regular updates on progression towards the agreed goals.
