Founded in 2010 by Jo Evans, Unite Professionals are one of the leading Case Management companies in the UK and provide individuals who have suffered life catastrophic and changing injury an opportunity to rehabilitate by advocating for and involving professionals working within various fields of rehabilitation to achieve a best possible outcome.

We primarily work within the personal injury sector for solicitors, insurers, private industry and paying clients and have a blended model of employed and associate Case Managers, supported by our Clinical Operations Team, skilled rehabilitation coordinators, customer liaison, finance and medical records teams.

We are proud to have a national team of field-based case managers covering all areas of the UK, with our central support teams being located at our Southport and Taunton offices. Our Case Managers are all clinically experienced Occupational Therapists and our Clinical Operations Manager, Rachel Culyer ensures they received regular supervision, are supported to engage in values-based and reflective practice with each and every client to ensure that Case Management remains client centred, independent, accountable and driven.

We offer three levels of Immediate Needs Assessment (INA) reports relevant to the clients’ injuries undertaken by one of our national and strategically located Occupational Therapist Case Managers. Once the Immediate Needs Assessment (INA) is authorised, Clinical Case Management is delivered by the same Case Manager to ensure consistency.
In addition, we also provide In-Reach Reports, Concise Updates, and Return to Work assessments.

Collaborative working is embedded throughout our working practices and our principle Vision, Mission and Values statement, underpinned by CMSUK and CQC guidelines reflects this. It supports our Case Managers and staff, and continued investment in our bespoke Case Management system, our customer relationships and our people ensures that we remain a leader in our field of work.

Return to Work Assessment

A bespoke clinically-informed assessment of the individual in the workplace to guide return to work planning.

The report will provide detail of workplace, job demands and the individual’s functional ability to meet these. Clear recommendations will be made to support a safe and sustainable return to work.

  • Complex Immediate Needs Assessment Report

    The INA report focuses on the impact of the injuries sustained on all areas of the client’s life, including personal care, leisure, transport, mood and work-related issues.
    • A clinically-informed bespoke assessment of immediate rehabilitation and support needs for complex major trauma injury, such as limb amputation, brachial plexus injury and moderate to severe brain injury.
    • Our approach is based on clinical interview, observation, and available evidence to inform clinical rationale and collaboration.
    • The report provides a clear account of the client’s treatment to date, baseline function and current function with costed recommendations to address identified needs for physical, psychosocial and vocational recovery.
    • The report will identify markers of progress, using client-centred goals and our bespoke outcome measure, the Areas of Impact on Function & Employment (AIFE).
  • Standard Immediate Needs Assessment Report

    A clinically-informed bespoke assessment completed by an experienced occupational therapist to identify immediate rehabilitation and support needs for major trauma injury, such as complex orthopaedic fractures.
    • Our approach is based on clinical interview, observation, and available evidence to inform clinical rationale and collaboration.
    • The report provides a clear account of the client’s treatment to date, baseline function and current function with costed recommendations to address identified needs for physical, psychosocial and vocational recovery.
    • The report will identify markers of progress, using client centred goals and our bespoke outcome measure, the Areas of Impact on Function & Employment (AIFE).
  • In Reach Support

    Our early intervention Report; In-Reach is for individuals who remain in hospital or a supporting rehabilitation centre, following the accident and who can benefit from early intervention and be assessed within the clinical environment.

    The ability of the Case manager to work with the current treating team to broker pathways to access local services, funding streams and potentially to complete a visit of the local area and home, especially if the client is to be re patriated from a trauma centre to their local area.
